The Federal Motor Carrier Safety Administration requires that interstate drivers of commercial motor vehicles maintain a current Medical Examiner’s Certificate. As part of the physical, you are required to fill out and sign the medical history portion of the Medical Examination Report form.

All of the forms necessary for your exam should be provided by your TeamCME Physician at the time of your appointment. However, if you would like to speed up the process, you can print and fill in the driver’s information and health history of your own copy of the DOT Physical Exam Report form by visiting the following link:

At the conclusion of your physical exam, you will be given the original copy of the Medical Examiner’s Certificate and a copy of your medical exam report form. The driver should provide a copy of the medical examiner certificate to their State Driver’s Licensing Agency and to their employer. If your employer requires a copy of the examination form, we suggest that you sign a release of medical information at the medical examiner’s office to allow a copy of the medical exam report form to be sent to your employer.
