This Medical Examiner's Certificate was formally released by FMCSA on December 4, 2018 and is the current version expected at this time to be remain unchanged for the foreseeable future.
TeamCME sells two versions of the 5875 MEC (11/30/2021). Commonly referred to as the "Stretched Version", this version of the self-laminating MEC uses the same height to width proportions as MECs that have been sold in the past and are available at other suppliers. Because this version does not strictly meet the FMCSA's and Office of Management and Budget height and width proportions and stretches the printed formatting, we refer to this version as "Not Fully FMCSA Compliant". However, this form has been sold by other suppliers for years and to our knowledge has never been rejected.
It features a laminated "original" copy for the driver, and a non-laminated copy for the medical examiner or motor carrier. This version is a tri-fold design for easy keeping in a wallet (slightly larger than a credit card). Two tear away tabs have instructions advising the driver to download the Free TeamCME Mobile App and to save your clinic as their medical examiner for convenient calling to your office and to receive text alerts from your clinic before their MEC expires. The other tear-away tab has instructions to the examiner regarding the "easy to perform" self-lamination procedure.

Note: Some providers and drivers prefer this version due to a wider area for printing and it's tri-fold design, however this "stretched" version do not strictly meet the specific requirements of the FMCSA & OMB approved MEC. At this time it appears that FMCSA is not strictly enforcing the MEC size requirements. TeamCME does not guarantee return of these card should FMCSA change their current position.
